Medical History Forms app is companion app from SurveysCreator.com, You would need to create a subscription based account at SurveysCreator.com to utilize the app.
Medical History Forms is primarily created as a flexible way for the UK Hospitals, Medical and Dental practices to get patient medical history using a tablet or phone, easily capture patient answers and get patient signature (requires a stylus based Android Tablet or Phone- Galaxy Note series is recommended).
Medical History forms along with the patient signatures are converted into PDF files and sent to your designated email address or stored in a local network drive.
You can change questions and the options for answers. The responses are stored securely, allowing you to run reports and gain key insights.
In addition to Medical History Forms, the app also allows capturing patient consents along with their signatures (with the facility to add witness signatures if required).
