Description of About Me (Autism Passport)

The About Me (autism passport) has been developed for those with a diagnosis of autism and also for those undergoing an assessment for autism.
The aim of the app is to explain and describe the assessment process. It will help the user to own and communicate their concerns when moving between different services and during transition. The passport will provide a summary of their assessments and contacts with different services. It also has a brief passport which can provide a profile of their difficulties and strengths.
This passport has been collaboratively developed with young people with autism and their parents/carers along with professionals working in this field. It can also be a useful resource for parents/carers and professionals working in this field.
