Health 360x (Health360x) is a patient engagement solution for self-monitoring and health coach intervention in chronic disease management. The application addresses gaps in implementation, and dissemination of evidence based care among populations most impacted by health disparities. Patients can self monitor and chart health data such as height, weight, medications, blood pressure and physical activity, and potentially share their data with the healthcare team.
