In 2013 NHS UK introduced a customer feedback mechanism called the "Friends and Family Test" (FFT). More recently this has been rolled out to GP Surgery's and other health establishments. This application satisfies the requirements of the test and allows you to collect the results any to forward to the NHS. Many companies are charging for expensive software solutions but this is free to use and helps you implement the test on any Android device. I ask only that you make a donation to the Children's Liver Disease Foundation (CLDF) based in the UK. Data from the survey is stored in /FFTData with a month qualifier and the previous months data is summarised at the start of the new month. The summarised data is what is required to send to NHS UK and the detailed data can be loaded to a text editor e.g. Notepad for analysis. Later additions will provide email capability and on-screen summary. Application is optimised for portrait mode.
