To make tomorrow better than today we have to learn. In practice a lot of experience comes from mistakes. Mistakes are good in terms of showing or questioning their root cause. After the root problem is fixed the mistake (and maybe other mistakes) will not happen again. To get to the level of root cause the Five Whys technique could be convenient.
