Do you have a Samsung phone and you get those blank icons when you start your phone or update certain apps?
Those are caused by TouchWiz (the home screen) not updating the app icons properly when the app is stored on an SD card.
Simply make a shortcut on your home screen to this app and run it whenever you start your phone or update any apps - or any other time you see those blank icons!
