It is Zooper Skin of Analog Clock showing date as well.
Keep widget size large for proper view.
Tested on Nexus 5 as well mdpi device.
HOW TO INSTALL
**This is a widget. No icon will appear on app drawer.**
Add it by long press on the home screen, choose "Zooper Widgets" from the pop-up menu.
Then resize grid size .
Go to template-sand select "analog clock".
